Civil - Structural - Transportation

 

 

Network: There is no calendar year deductible. Coinsurance is 100%. Physician office visit is $10 copay per visit, then coverage pays 100%. Emergency room services has a $150 copay, then coverage pays 100%. Immunizations, second surgical opinions, allergy testing/treatments, inpatient facilities, and several preventative services are covered 100%.

Non-Network: Calendar year deductible is $300/single and $600/family. Coinsurance is 70%. Physician office visit is $15 copay, then coverage pays 70%. Emergency room services have a $150 copay, then coverage pays 100%. Second surgical opinion and several preventative services are covered 100%. Immunizations, allergy testing/treatments, and routine physical exams cover 50% after deductible.

Prescription Drugs: Prescription drug copays are $10, $20, and $40 depending on program. There is also a home delivery program that offers savings as well.

Dental Insurance: Maximum benefit is $1,000 per person, per calendar year. Preventative procedures requires no deductible and the policy provides 100% coverage. Basic procedures involve a $50 deductible and coinsurance of 80%. Major procedures, such as dentures and bridgework, have a $50 deductible and coinsurance of 50%.

Personal Days Program: This program offers 15 to 25 paid days off each year, depending upon each individual's years of service with NCI. Time off for holidays or disability is not included in this program.

Holidays: NCI normally observes seven (7) holidays in which the office is closed and employees are paid: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Eve Day, and Christmas Day.

Short-Term Disability: Benefits begin on the 1st day of disability due to injury, and the 8th day of disability due to illness. You will receive 60% of your weekly pre-disability earnings, up to a maximum of $500 per week. Maximum payment period is 13 weeks.

Long Term Disability: Benefits begin after you have been disabled for 3 months. They equal 60% of your monthly pre-disability earnings, up to a maximum of $5,000 per month. Benefits last as long as you qualify as disabled, to a maximum of your Social Security Normal Retirement Age.

Life & Accidental Death & Dismemberment Insurance: Benefit of $50,000.

401K Retirement Plan: Each qualified employee may contribute between 1% and 60% of his or her salary to the plan, up to the annual IRS dollar limit. NCI donates a variable partial matching contribution.
